FEES
A. The appropriate application fee outlined in 7.701.4, C must be submitted to the Department with the application for a child care license at least 60 days prior to the opening date of the facility or the expiration date of the provisional or probationary license.
B. The appropriate application fee outlined in 7.701.4, C must be submitted to the Department annually, at least 60 days prior to the anniversary date of the license, along with a completed continuation declaration.
C. Following is a schedule of annual fees for all types of child care facilities:

| Family care home (excluding homes certified by counties or child placement agencies) | $16 |
| Large Family Care Home (7 to 12 children) | $ 24.00 |
Annual Licensing Fees for Public, Non-Profit, Proprietary or Commercial Facilities
(Includes all facility types except Family Care Homes and Child Placement Agencies)
A Full-Day Program Operates 6 or More Hours a Day. A Full-Year Program Operates 12 Months Out of a Calendar Year. A Part-Day Program Operated Less Than 6 Hours a Day. A Part-Year Program Operates Less Than 12 Months Out of a Calendar Year.

The annual report required by regulation 7.710.72 (B) must be attached.
D. The following is a schedule of fees for changes to licenses and for duplicate licenses:
