On the state website an amendment to the state investigation has been posted. The original investigation is not and will not be posted.

This is in regards to the 5 month old who passed away at daycare.

So it looks like unsafe sleep practice was the cause of death?

Yes. She slept him on an adult bed instead of in a pnp or crib. So it looks like unsafe sleep practice was the cause of death?
How sad, completely preventable.Absolutely preventable. I don't understand why people, providers, continue to use unsafe sleep practices.
