Default Formula mixing policies

What are the VA state policies for mixing formula at a daycare center? And what are the policies about have a premixed formula/rice bottle for your child? Thanks.

Default Baby Formula at Daycare

I can find no requirement on mixtures. This is all that I found:

Prepared infant formula shall be labeled with the individual child's name and kept in the refrigerator when not in use.

22 VAC 40-110-1010. Formula preparation.
If infant formula is heated in a microwave oven, precautions shall be taken to prevent scalding. Only refrigerated formula shall be heated. When formula is heated in the bottles, the bottles shall be upright and uncovered. Heating time shall be no more than 30 seconds for four ounce bottles and no more than 45 seconds for eight ounce bottles. After heating and replacing nipples, bottles

Default formula preparation

baby formula and expressed breast milk should NEVER be microwaved as the risk of scalding due to uneven heating is too great. Expressed breast milk can also lose nutrients during microwave heating. The water used for heating the bottle may be microwaved,with the bottle then being placed in the heated water for several minutes.
