Application

  

        (a)  The application for license shall be made on    

forms supplied by the department and shall be completed in    

a manner prescribed thereon.     

        (b)     Applicants shall provide such criminal history,    

background, employment information, and consent to conduct    

such checks as may be required by state or federal law.     

Such information and consent shall be given upon forms    

supplied by the department.     

        (c)     The department shall conduct employment history    

and background checks on all applicants and background    

checks on all prospective employees; applicants shall    

conduct employment history checks on prospective    

employees.
