Evidence of Child's Health
(a) The facility shall require and obtain from the
parent or guardian of each infant and toddler entering
their program a health record of the infant or toddler,
which consists of the following:
(1) Written evidence of a physical clearance
obtained within two months of admission to the facility;
(2) Written evidence that the infant or toddler has
received a tuberculin test indicating that the infant or
toddler is free from tuberculosis in a communicable form;
and
(3) Initial and continuous written evidence that
immunizations are current; or
(4) A written statement from a licensed physician
certifying that the physical condition of the infant or
toddler is such that immunizations would endanger the
infant or toddler's life or health; or
(5) A written statement from a parent or guardian
requesting exemption from the required immunizations on
the grounds that such immunizations conflict with the
parent or guardian's bonafide religious tenets and
practices.
(b) Medical clearance for care of children with
special needs shall be obtained before admission into an
infant and toddler facility. Special needs children mean
children with any condition which requires modification of
care or services not regularly available.
(c) The records of the infants and toddlers in the
program shall include pertinent information about birth,
health status, developmental progress, including
prematurity and any special needs and efforts necessary to
meet these needs.
