Information on Owner or Operator

  

        (a)  The name, address, and telephone number of the    

facility shall be supplied to the department.    

        (b)     The name, business address, and business    

telephone number of the persons having authority over and    

responsibility for the overall administration and services    

shall be supplied to the department.    

        (c)     The name of the owner or sponsoring agency    

(privately owned, church, or agency owned, etc.) of the    

facility shall be supplied to the department.
