Illness Log
EC (a) For early childhood programsx "early childhood programs:illness
log", the following shall apply:
- The center shall maintain on file a log of the initial illnessesx
"children's records:illnesses"x "illnesses:records", symptoms
of illness or diseases that are exhibited by each child while in the center's
care, as specified in N.J.A.C. 10:122-7.1(c) and (d). This illness logx "illness
log" shall include:
- i. The child's name;
- ii. The date and time the illness, symptoms of illness or diseases
were observed at the center;
- iii. A description of the symptoms of illness manifested by the child;
- iv. The action taken by the center to assist:
- (1) The child who is demonstrating symptoms of illness; and/or
- (2) The director in determining if exclusion of the child from
the center is necessary;
- v. Any significant change in the child's symptoms of illness; and
- vi. The date, if applicable, that:
- (1) The child was removed from the center;
- (2) The child returned to the center with a licensed physicianx
"physician:authorization:admission of sick child"'s note
attesting to the child's admissibility to the center and recovery
from a reportable disease, as specified in N.J.A.C. 8:57;
- (3) The child returned to the center with a parentx "parent:permission:admission
of child with chicken pox"'s note attesting that six days have
elapsed since the onset of chicken poxx "chicken pox", or
that all chicken pox sores have dried and crusted; or
- (4) The child returned to the center symptom-free.
