R9-5-704
Renewal of certification
A. An application for renewal of a day care group home certificate shall be submitted on forms provided by the Department no more than 60 days, but not less than 30 days, before expiration of the current certificate. The renewal application shall be accompanied by a non-refundable fee and all required forms and inspection reports specified in R9-5-701 (B).
B. When a completed application for renewal of certification is submitted prior to the expiration of the certificate, the existing certification shall remain in effect until the issuance of a final Department decision regarding that application for renewal of certification.
