I don't know where to start with this one. I already have my rates in mind, but would anyone mind showing me their format?

Not sure if you mean a rate sheet or a contract page outlining what parents are going to be paying.

Either way here is a form I have used in the past. If you want, you can use any or all of it if it works for you. :)
