Hazardous Protection Plan

Hello everyone, we are trying to complete our licensing application for a home based day care here in Chicago, IL. One of the requirements as many of you know is the Hazardous Protection Plan. Wondering if there are any samples we can use to complete ours. appreciate any help. I believe AZRA11 posted a copy but I can not access it.
