Preschool/Pre-K Form
 
I looked through the form area and can't find what I'm looking for and may need to make my own up as I can't find anything on the computer either.

Looking for a form to have parents to fill out the beginning of the school year (even though I have kiddo's all year) with what their child knows/can do.

Like:

Can they cut using scissors
Do they hold a pencil/marker/crayon with traditional pencil grip
Have they used glue before

Etc.

Anyone have a form like this or know where I can find one?

Don't you do assessments upon enrollment?

I do an assessment on each new child upon enrollment so I can set realistic goals for progress through out the year.

Any information a parent provides always seems to be "off" in comparison to what the child really CAN do. kwim?

I've had parents tell me their child can read/write and speak in complete sentences but at daycare, they can barely utter their names in an audible form.

I don't because parents do NOT accurately represent their children.

I make my own observations and move on from there.

I do my usual Ages and Stages as well. I like this one because it engages the parents. ;) It is letting them know what will be expected. I like to have them fill one out too, then compare it to my own. It is often entertaining and let's me know what I am up against. :lol:

Actuality: Has been able to for the past year

Yet at the same time..
Does child know his colors?
Parent: yes!
Actuality: Only if everything is blue...
