I think this is when it is a tough call. "Symptom free" for 24 hours would mean (to me) that all spots and signs of reaction need to be gone for 24 hours before returning.
I would probably ask the mom what the note says. If it says this is just a reaction to shots, he would need to stay home until symptom free. If it says he can return to daycare, then I would let him return, with a reminder to mom that if he is acting more clingy, whiny, lethargic ...than normal, she will be getting a call for immediate pick up.
