I just got this confirmed with my licensing analyst. This is EXACTLY what she said as well.

I was concerned b/c Crystal claimed there was a form for this and I had never heard or seen it. Because I do transport to and from school this was extremely important to me to get the facts straight!

In regards to the OP, I understand why you are upset. I would NEVER have transported a child w/o a parent giving me permission to do so. However, since CA does not have any kind of written consent forms regarding transportation and as long as the provider used the proper car seat and followed the CA laws regarding using them, licensing will not have a reason to cite her per Title 22 regs.

I hope you will be able to find a new daycare that better fits your needs.
