I think a pre-registration form is a great idea. I don't like the idea of just having them look over stuff at the interview and not giving anything to take home. I use those folders from Staples that you can get during the 1¢ sales.

Do you give out references? To maybe make your packet look a little complete I would probably include the pre-registration form, the universal healthcare form (assuming you are in the US and use it), your references and maybe something "fun" like your daily sheet or your daily schedule.

Then when they bring that back with the deposit give them your "real" packet.

Heck, if I can get myself to revamp my forms I think I'm going to steal this idea!!!
