You can print blank W-10's to fill out on your own. The form is both free online (
http://www.irs.gov/pub/irs-pdf/fw10.pdf ) and available to print in MMK forms.
If you need the total and parent info pre-printed then you will have to put in every payment the parent made first.
In your shoes I'd do this years from the records I already have and start next years, now.... No need to stress.
