I have had the "left behind sleeping baby" nightmare many times.
It has various endings, none good. I find it happens more often when I have too many chronic "last minute" pick-up families in care. More specifically, when I have to rush out the door behind them, to my second job, too often because they waited until the last minute.
It has improved greatly since changing hours to allow a 30-minute buffer.
