I dont have a form, but this is what I send to the parents when they have broken one of the PHB rules. After this letter, I then term. I have only had to term twice.


Prior to your child’s admission to our day care, we entered into to an agreement to follow all rules of the parent handbook and its policies. Below is a reminder of arrival and departure policy.

I then state the exact incident and then let them know that if it happens again that I will term their contract without refund.
