Please read over the attached forms and contract. Your signed contract and forms are due back no later than October 26th, 2012 and will be effective on NOVEMBER 5, 2012. If I have not received your signed contract on by this date (October 26th, 2012 by 5:00pm), I will assume you are no longer interested in my services and child care will be terminated on NOVEMBER 5, 2012.
