I have had kids poop 3-4 times a day and multiple diaper rashes as well. No blowouts though! My own 5 yr old poops 3-4 times a day, eats like a horse and is only 37 pounds!!

Might be some sort of sensitivity, lactose comes to mind or possibly a parasite. I would ask them to get a Dr visit at minimum.
