I need an "incident report" form. My 2 yo bit my 4 yo today - first time I've had one dck bite another.

I have an accident form for the one who was bitten, but I need an incident for the biter.

(Yes, I am too lazy today to hunt for one.)

I use the same form! I think it's called oh no report?! I have only had to use it a few times.
