Hi guys! I'm wanting to hand out a form for the parents to fill out about their children, things like Full Name, Birthday, nap time routine at home, favorites songs at home etc. Does anyone have one they wouldn't mind sharing? Or just suggestions of information you ask before a child starts? I don't want to miss anything important. ( I promise I did search the forum first, but I'm just finding posts about using a form like this, not the actual form )
