Does anyone know the number I can call to order the blue immunization record forms?

Blank PM 286B forms are available from either:
(1) the California Department of Public Health (CDPH) by downloading it for printing (white paper is OK; it doesn't have to be blue) at http://www.cdph.ca.gov/pubsforms/for...ms/pm286b.pdf; or from:
(2) your local county health department (You can find a listing of county health department phone numbers via a link on our website at http://www.pheofca.org/immunizations.html. Your county health department’s phone number also will be in the county section of your phone book.) When calling, tell them you are a private school administrator from (name of your school) and you need forms for complying with immunization requirements.

Did this change recently? My analyst requires it printed on blue paper.

The first link I posted says "It is suggested that this document be printed on blue paper"

I am not familiar at all with CA rules for this...I just did a quick Google search and those links are what showed up.

Ahh .. ok, gotcha .

I was able to order mine from the county health dept. They mailed me about 50.

ditto..

if you call your county health dept they will help you with anything to do with health related issues.

I got all kinds of cool stuff from them for free. I got cool stickers, wellness posters, posters to hang that talk about when to exclude children from care. I love that one the most because when a parent tries to say anything to me, I walk over to it and say, lets see what the county says about that.
