Does anyone have a preventative basic care items form that I can view . This would be for things like sunscreen , diaper cream , lip balm etc...

I was told that I now have to have a form authorizing me to be able to use these items on a child . This is for non prescription items .

Mine is simple. I haven't updated mine in a while but I probably will someday to say something about how parents needs to provide whatever topical product they want me to use. My policies cover this though so I haven't felt an urge to add it to my form yet. HTH

Does your licensing provide medication forms? I just use the one from community care licensing that's meant for prescription and OTC meds. I put sunscreen and diaper cream on the OTC section.

We mark the end date as "end of care and/or when medication expires"

heres what we use

but we dont write down everytime we put on butt cream or sun screen, or lip balm. We do write down any other medication including the ones we have to daily
