I remember you've suggested we have parents sign their yearly W-10. Can you please suggest wording for that statement?

The W-10 form is a pre-printed form from the IRS

I just have the parents sign their name and date it near the top of the form.

Easy peasy.

Thanks blackcat. I have the form. Tom has suggested wording that we print at the bottom of our accounting that states the parent accepts this as an accurate account and won't claim more to the IRS.
