For the ones who do give parents a tax form besides the IRS
one. Can you share with me what website you get it off of or what yours may look like. I am looking for one that both myself and a parent has to sign. I am having a heck of a time finding one.

It's just a regular receipt, with my EIN on it. I write "year end child care statement" in the space for what it's for.

That was before kidkare. Now I just use the W-10 that kidkare generates for me.

I just typed up my own. It says “child care payments for 201X” on the top.

Providers EIN or SSN:
Parents Name(s):
Children’s Name(s):
Total Tuition Paid:
Parent’s Signature:
Provider’s Signature:


I’m actually giving out a 2018 one today already as I have a daycare girl moving out of state

I just use a W-10. There is a space on it for the provider to sign/date and I just have the parent sign/date at the top of the form.
