Does anyone have an interview form? I'm looking for something that I can use when meeting with the potential clients.

I don't have a "form", but there are only 3 key things I always ask of parents.
1. Has your child been in child care before? What type and for how long?
2. Any medical, emotional or nutritional issues I need to know about?
3. Last question is always asked AFTER I have reviewed my policy statements "Do you have any questions or comments about the policies we just discussed?".
Maybe others will not agree, but when I conduct tours of my center, I always allow the parents to 'interview" me. I welcome questions about experience, education, philosophy, curriculum, the daily schedule and policies. I don't believe in having a long list of questions for parents that can be construed as a "grilling". My opinion. I prefer to be more of a salesperson than a detective.
