Does anyone do drop in care? Do you have a form that you have the parents fill out?

I am going to offer drop in care for the first time in over 9 years. I never liked it when I did it, but I have a situation where the family just moved here, is looking for a job and wants to have the child drop in until they get employed. At that time they will then come full time.

I want to use a form, but don't want to have to create one....lol

I do drop-in. I have clients sign a drop in contract and follow my policy handbook just like other clients. Here's mine.

Drop-In Contract - must be logged in to Google account to see.

I love your contract, but I am looking more for a form.

for example a form to be used to reserve the days/hours needed.

Monday 8-5- 9 hours Fee $90.00

Tuesday 9-4 - 7 hours $70.00


Total due is $160.00

Like a statement? I use MMK for that but Word has some you can use. Not sure if that's what you mean or not. Like a breakdown of the days they reserve and the total for those days?

oh that sounds good.

basically what I am looking at is something that they fill out that commits them to their days requested. Like a reservation form.

I have one but I can't download at work.




It has my title

Then says :

No child will be admitted for drop-off care without payment.


Emergency drop-off care will be provided at _____ rate due at drop off, but is dependent on space available.

Drop off days are limited to _______ hours per day at the above listed rate. Any additional hours are _______. Late pickup fees of _______ apply.


All drop-off clients are required to follow rules and regs as set forth in handbook.


Provider reserves the right to refuse drop off care for any ill child and advanced booking will not receive refund unless 24 hours notice is given.

Drop In
Drop in care will be provided at a rate of $12/hr

Drop in care will be as space is available

It is the responsibility of the parent to confirm with the provider their child’s drop in schedule prior to the day of care (up to one week in advance).

Tuition will be due on a weekly basis on the first day of the week care is provided (Ex: care is scheduled for M, Tue and Th, tuition for the whole week of care is due on Mon).

Drop in clients are not responsible for provider holidays, vacation days or sick days.

Drop in clients are responsible for tuition if their child becomes sick prior to the day that care is scheduled for or if they become sick during the day and need to be sent home. (Ex: parent confirms drop in care for Tue and Th, child becomes sick Monday and is not able to attend care on Tue and/or Th. The client is still responsible for tuition for Tue and/or Th.)
