I have a 2 1/2 YO DCG who for some time has been having some major problems with her bowels. She is either really constipated or withholding her BMs. Either way she spends a good part of the day grunting in a corner and crying. (It is heartbreaking to listen to as there is nothing I as a provider can really do to help her)
But the main issue I have is that fact that she has been squirting poo every 15-20 minutes all day, every day and it smells horrid!! Her diaper rash is so bad it pains me to wipe her (Some areas almost have open wounds...) DCM has given me numerous creams for it but this DCG screams bloody murder every time I try to clean her up and it is constant because she is constantly squirting out poo.
DCM says to only change her every 2 hours or so unless she finally does something decent but I can't stand the smell and the other DCKS don't want to be near her and complain about her odor.
I told DCM she needed to take DCG to a Dr. and she did. She came with several different creams and told me DCG is being put on stool softeners...
Not sure what else to do, I have nothing about something like this in my contract.
Would you exclude?

When was that last appointment? are the creams or softeners working at all?

Mom only wants her to be changed every 2 hours and she's pooing every 15-20 minutes? Seems like that would make it worse! Poor baby!
I would need to see a Dr. note or something. It just doesn't sound right. I take it she's constipated, but the stool softeners are making her go every 15-20 minutes? Sorry if I am misunderstanding what you said, but I'm confused.

It sound slike she had a blockage. I would insist on a specialist at this point. It is painful for her and the smell and constant one on one can't be good for the other kids.

And, no way would I let her sit in a dirty diaper for two hours. Every two hours for wet diapers maybe, but change the BMs immediately.

Um, no way....Sorry mom....the child gets changed when necessary. If mom wants things to be any different she can stay home with her then.

Parents can NOT give you permission to do the wrong thing and leaving a child in a soiled diaper, pull-up and/or undies is NOT okay ever.

It sounds like she may have developed a fistula with the leakage you describe.

I'd tell mom it's against licensing protocal to leave her sit in her own waste and you are concerned not only about the waste making the rash worse but about the smell possibly indicating infection.

If it had been a week or more since she was last in I'd insist she bring her back. That or call for pick up every single time she disappears into the corner in pain.


Geez Louise I treat my DOGS better than that......

wonder if she has a food intolerance, particularly gluten or dairy. They produce really horrid smelly poops, usually diarrhea if intolerant. Poor thing, & poor you for having to deal! I'm also wondering if she has candidiasis, as the yeast can cause a nasty, open, hard to cure rash.

Also, yeah, if she's poopy, change her. I don't get what mom's thinking..Let my kid sit in poop and make that rash worse so I can save $ on diapers?!

I was thinking the same thing. And I change diapers when they NEED to be changed. I understand they are expensive, but that's part of having kids. A full diaper or ANY poop in a diaper gets changed. I do 5 changes a day on a schedule, and anything that needs it in between. Mom MUST get that kid to a pediatric gastroenterologist.

My first thought was food allergy. My son had these. It was horrible!!! Pooping every hour if not more, horrible rashes with open wounds, super stinky. It lasted weeks. I put him on a super clean diet: Plain oatmeal, crackers, plain chicken and eggs, applesauce, etc. Then one day it cleared up. When it came back I immediately looked at my menus and we had sweet n sour meatballs (it is loaded with pineapple). It stopped then happened again which I deduced cantelope. Same thing! These were two staples!

If you have the time (I know that is a joke!) talk to mom what she is serving at home versus your care and maybe you can figure it out. Or give her a super clean, plain diet for a week and see if it clears up.

What you are describing (the constipation with squirting) can be very dangerous, especially for a small child. It needs to be cared for. If you suspect this mother really went to the doctor, and is actually on the stool softeners...maybe.
If you at all suspect she is full of it, I would exclude until the problem clears up. And no, leaving her in poop for 2 hours to save diaper money, especially in a child with a rash already. Not ok.
If that has been happening at home, and she has open sores,she may have a yeast infection.

I posted about this in another thread, but I was watching a 6 year old with the same "squirting poop" problem. Usually what happens is a hard mass of poop develops when children are constipated for a long period of time. Therefore, the only poop that can pass by this mass, turns into sludge- and it really stinks! The boy I watched never even knew it was coming out. To this day, his mother has done nothing to help him (he eats only processed foods/juice, etc...). They say when it gets to this point, a change in diet/ laxatives don't help. You really need to have an x-ray to determine if a "clean out" surgery is needed.

yes i would ask mom to bring her back and this time get a note (someone posted a dr's note here that they send) also, maybe try sitting her on the potty sometimes this will help too.

but I agree with the blockage thats why she is seeping. I know some won't agree, but maybe if you put vaseline on the open areas it will act like a barrier so the poop won't touch the skin
