I need your help with some ideas or examples for program evaluation survey's I need to give out for parents to fill out regarding the program.

Is this something you want to do just because.... or is it something that is required by licensing or your state stars program?

Hi Dave! Long time no see!

I do a rating system 1 to 10.
Please rate satisfaction with:
Cleanliness
Curriculum
Arts and Craft
Communication with provide
Clarity of policies
Overall satisfaction

I also ask them to name things they like about the program, things they would like to see improved.
