Does anyone have a form they can send me from Child Care Resource and Referral that has authorization for prescription medications? I have one for OTC medications, but not prescription and would really like a professional looking form from child care resource and referral. I'm trying to look online but I can't find it yet. If you have one please post a link or attachment. THANKS

Thanks for the help. I was able to find my local one didn't take long at all and glad I found it. I think* I have all my "possibly" needed forms printed and on hand now. That will come in handy!
