If parents ask me for my social do i request this form or just give them my SS#? I am a family childcare in CA Los angeles County please help

We have a Tax ID we give parents in OK.

You can file for an EIN number which is very simple to do and free. It makes it so that you no longer have to use or give out your social security number.

The W-10 form is something the parents are suppose ot bring to you to fill out out. It doesn't have any yearly totals on it unless you use the one from the MMK software. You are not required to supply parents with a yearly total or with a W-10 form but most providers do as a professional courtesy and since it is so easy to print out if using MMK, it is no big deal.

Yes, use the Form W-10 as a sole proprietor.
