will send my son for only 3 weeks to a registered home DC. In the interview the provider didn't speak about signing a contract or filling any forms. I will just give her his vaccination chart. Is it enough? Do you sign a contract if it is short period? She also said she can take him out in nearby park, but I am not comfortable what should I do?

I think that a contract is appropriate, no matter how short the time of care is. I know that some providers choose not to use them though. As far as forms, in your state it may even be required by law that you provide your day care provider with forms of some variety, especially health and emergency contact information. If she's registered, I find it a bit odd that she didn't mention it. Maybe you should ask her?

Regarding taking your child to the park - if this is something that the provider does on a regular basis with the other kids, and you're not comfortable with it...the only solution is to find another provider that doesn't do that.
