I know I saw a thread about what to include in the new child enrollment packet, and now I cant find it. :-(. What forms do ya'll include in your enrollment packets? Thanks in advance :-)

Here in Illinois we have to include a summary of DCFS licensing standards that they have to fill out the back page and return for my files. I don't know if TX has something similar.
