Step 1 - Take the online Licensed Application Process (LAP) training To access the LAPS training, you must have a log-in for the NEATS system. If you do not have a log-in, view the PDF below for instructions to create one. Applicant and Owner/Director must sign third page of document. Incomplete forms will not be accepted and will be returned.

Step 5 - Upon completion of fingerprinting a copy of the Consent and Release form and applicable work card(s) must be sent to Child Care Licensing for audit purposes. |

* Ratios shown in table are calculated based on the maximum number of children allowed to one caregiver. Center classrooms with children over 2 years of age but under 3 years of age are required to have a child:staff ratio of 10:1. Otherwise, the ratios in child care centers are based on the total number of children in care, not the age of the children. Requirements for children age 2 and older are as follows: For every additional 13 children above 93, one more caregiver is required.
